The nurse practitioner role in CKD disease management in the NT : building on the lessons learnt by others
The area of chronic disease management is an ongoing challenge and the role Nurse Practitioner in case management of chronic disease clients has the potential to improve services directly to clients. The NT is one of the last states and territories to implement the Nure Practitioner role and therefore has the opportunity to learn from those who have gone before.
Objective: By undertaking a small qualitative study of Nurse Practitioners within Australia working within the renal and diabetic specialties of chronic disease management the author will identify the barriers and enablers to the implementation process and using the results formulate an implementation framework for use in the NT preventable chronic disease programs.
