The Chronic Kidney Disease Clinic Pilot Site – Reporting 2 years’ results
The CKD Clinic at Princess Alexandra Hospital opened in April 2006, as the Queensland Health pilot site for a multidisciplinary approach to Chronic Kidney Disease (CKD) patient care. This created not only a new model of care, but also a new advanced practice role for nurses.
Evidence based guidelines have been used to develop individualised patient treatment targets and manage risk factors, with the aim of reducing complications of CKD, and avoiding or delaying the need for dialysis and transplantation. The main intervention targets in this group of patients were identified as blood pressure control, blood glucose control, proteinuria, lipids, mineral and bone disease, and anaemia management.
As the nursing role has previously been discussed this will not be re-examined, but we are now considering if this model of care has achieved its goals. We will present the results of nearly two years experience of this model of care. There will be discussion of factors which have contributed to successful outcomes and lessons for future practice.
